Terms of Purchase

PROFILE CREDITS

‍

  • A Profile tested during a billing period will count as a Profile Credit for that month. A Profile Credit includes unlimited testing for that Profile within the billing period.
  • Profile Credits never expire, unless customer cancels their subscription.
  • In the event you go over your pre-purchased Profile Credit amount, you will be billed at the end of each billing period for the excess number of Profile Credits used.

BILLING PERIOD

  • Billing periods are monthly, beginning on the date in which service is activated and will continue on that day each month after. For example, if service is activated on January 4th, your next period will begin on February 4th, and so on.

MONTHLY SERVICE FEE

  • A monthly service fee of $25.00 will be billed to your account every billing month.

AUTOMATIC BILLING

  • By inputting credit card information, I hereby authorize Sway Medical, Inc to charge the credit card information provided pursuant to (a) the terms and provisions of the quote and agreement and (b) any related agreements with Sway Medical, Inc. This authorization is to remain in full force and effect until Sway Medical, Inc. has received written notification of its termination in such time and in such manner as to afford Sway Medical, Inc. reasonable opportunity to act on it. I acknowledge that I am authorized to provide and submit this information.

QUOTE EXPIRATION

  • This quote is only valid up until its expiration date, at which point the price can be subject to change.

SALES TAX

  • Sales tax is not included in this quote, but may be applicable in your state.

PRICING

  • Sway Medical reserves the right to adjust pricing. Customer will be given minimum 30 days notice before any pricing change takes effect.

CANCELLATION

  • In the event that a customer cancels their subscription, any unused Profile Credits will be voided. If a customer renews their subscription after a cancellation, the customer must repurchase new Profile Credits.
